NOTICE TO THE PUBLIC

Background information has been provided to the board on all matters listed under the Consent Agenda, and these items are considered to be routine by the Board.  All items under the Consent Agenda are normally approved by one motion.  If discussion is requested by any Board members on any item, that item will be removed from the Consent Agenda if separate action is required.

 

Agricultural Commissioner/Sealer

3.            Approve a capital asset expenditure of up to $130,000 for roof repairs on the Agricultural Building in Tulare.  Approve the necessary budget adjustments (4/5ths vote required).

Board of Supervisors

4.            Appoint Supervisor Pete Vander Poel, Alternate Member to the Local Agency Formation Commission for a term ending the first Monday in May of 2019.

5.            Appoint Crystal Sullivan to the Chairman, Board of Supervisors (or designee) seat of the Law Library for a term ending December 31, 2015.

6.            Appoint Dr. David D. Wood, Seat #5 to the King/Tulare Area Agency on Aging Advisory Council for a term ending December 31, 2016.

7.            Reappoint Melissa Prado, Parent/Consumer and Dianne Young, At-Large Member to the Tulare County Child and Youth Development Council for terms ending June 30, 2017.

County Administrative Office/General Services

8.            Ratify actions by the County Administrative Officer during the month of May 2015 regarding Good Works agreements and Work Alternative Program agreements.  Find that the activities funded pursuant to the good works funding agreements are necessary to meet the social needs of the residents of Tulare County.

9.            Approve an agreement for Purchase of Fee Title and a Temporary Construction Easement from Giumarra Bros. Fruit Company Inc., a California Corporation, for the Avenue 416 Road Widening Project in the amount of $667,000.  Authorize County Counsel to sign the Grant Deed and Temporary Construction Easement acceptances upon presentation.  Authorize the Property Manager, or designee, to open an escrow and sign all documents to facilitate the escrow for this acquisition.  Direct the Auditor to draw a warrant in the amount of $670,400 in favor of First American Title Company.

District Attorney

10.         Approve an agreement with the California Governor’s Office of Emergency Services in the amount of $86,323 to fund the Human Trafficking Advocacy Program, effective October 1, 2015 through September 30, 2016.  Authorize the County Administrative Officer, or his designee, to sign the Certification of Assurance of Compliance.

Fire Department

11.         Authorize the submission of a grant application for a Department of Forestry and Fire Protection Volunteer Fire Assistance Grant to purchase personal protective clothing and equipment in the amount of $40,000.  Authorize the Fire Chief, or his designee, to sign and submit the grant application.

Health & Human Services Agency

12.         Approve the credit application agreement with AmerisourceBergen Drug Corporation for pharmaceutical distribution services with Walgreens.

13.         Authorize the acceptance of $150,000 from Kings County as a result of the cost share agreement for Suicide Prevention Task Force operations and continuing projects for Fiscal Year 2014/2015.  Approve the necessary budget adjustments (4/5ths vote required).

14.         Approve amendments to agreements with the following school districts:  Agreement No. 24417 with Tulare City School District; Agreement No. 24416 with Tulare County Superintendent of Schools; Agreement No. 24421 with Woodlake Unified School District; and Agreement No. 24418 with Tulare Joint Union High School District to receive immunizations and public health services from Tulare County Health & Human Services Agency and to extend the date of termination, effective July 1, 2015 through June 30, 2018.

15.         Approve an amendment to Agreement No. 23277 with Kings View Behavioral Health Systems to update Exhibit B which will increase the agreement by $700,000, from $1,290,000 to $1,990,000 for the provision of a Methadone Maintenance Program, retroactive to April 1, 2015 through June 30, 2016.  Find that the Board had authority to enter into the proposed amendment as of April 1, 2015, and that it was in the County’s best interest to enter into the amendment on that date.  Approve the necessary budget adjustments (4/5ths vote required).

16.         Approve an amendment to Agreement No. 24233 with the City of Porterville to extend the term of the agreement to June 30, 2016 for the Recycle-Only Household Hazardous Waste Program.

17.         Approve an agreement with Cutler-Orosi Joint Unified School District for the provision of a Family Transitional Supportive Services Program in the amount of $119,217, effective July 1, 2015 through June 30, 2016.

18.         Approve an agreement with Family Services of Tulare County in an amount not to exceed $355,190 for the In-Home Parent Education Program, effective July 1, 2015 through June 30, 2016.

19.         Approve an agreement with Pleasant View Elementary School District for Tulare County Health & Human Services, as the Local Government Agency, to act as the claims processing agent of Medi-Cal Administrative Activities claims on behalf of their school district, effective July 1, 2015 through June 30, 2016.

20.         Approve an agreement with Starview Behavioral Health, Inc. in an amount not to exceed $280,000 to provide Community Treatment Facility and Psychiatric Health Facility beds and related mental health treatment for Tulare County mental health consumers, effective July 1, 2015 through June 30, 2016.

21.         Approve an agreement with Central Star Behavioral Health, Inc. to provide youth acute inpatient Psychiatric Health Facility beds and related mental health treatment for the benefit of Tulare County youth consumers in an amount not to exceed $250,000, effective July 1, 2015 through June 30, 2016.

22.         Approve an agreement with Family Builders, Inc. in an amount not to exceed $300,000 to provide Therapeutic Behavioral Services, effective July 1, 2015 through June 30, 2016.

Resource Management Agency

23.         Prohibit motor vehicles from stopping any time on the south side of Avenue 128 (Teapot Dome Avenue) in front of Hope Elementary School by establishing a “No Stopping Anytime” zone.  Prohibit motor vehicle parking on the south side of Avenue 128 (Teapot Dome Avenue) in front of Hope Elementary School by establishing a “No Parking” zone by painting red approximately 205 feet of curbing along the front of the school’s parking lot and 40 feet of curbing between the two east side driveways.

24.         Adopt the Categorical Exemption prepared pursuant to the provisions of the California Environmental Quality Act (CEQA) for the Asphalt Concrete Overlay Projects at the following locations:  1) Road 200 from Avenue 0 to Avenue 8; 2) Road 184 from Avenue 136 to State Route 190; 3) Diagonal 208B from Avenue 160 to Road 208; 4) Road 148 from Avenue 280 to Avenue 288; and 5) Avenue 432 from State Route 63 to Road 136. Authorize the Environmental Assessment Officer, or designee, to sign and file the Notice of Exemption with the County Clerk. Approve the Plans, Special Provisions, Proposal and Contract (“Bid Documents”) for the construction of the Asphalt Concrete Overlay Projects.  Approve the advertisement for bids of the Asphalt Concrete Overlay Projects.

25.         Approve and award the contract to the lowest responsive and responsible bidder, Witbro Inc. DBA Seal Rite Paving and Grading of Fresno California in the amount of $1,018,932 for construction of Reservation Road (M137) Safety Improvement Project. Authorize the Chairman to sign the contract, subject to review and approval as to form by County Counsel and upon receipt of certificates of insurance, bonds and evidence that the contractor possesses a valid State Contractors License.  Allow funds for contingencies in the amount of $102,000 (10%) to cover unexpected construction conditions.

26.         Approve and award the contract to the lowest responsive and responsible bidder, Agee Construction Corporation of Clovis, California, in the amount of $1,599,909.30 for construction of the Asphalt Concrete Overlay Projects at the following locations:  1) Avenue 272 from Diagonal 88 to Shirk Road; 2) Avenue 256 from Road 164 to Road 180; 3) Diagonal 112A from Avenue 184 to Rankin Avenue; and 4) Road 208 from Avenue 112 to Teapot Dome Avenue.  Authorize the Chairman to sign the contract, subject to review and approval as to form by County Counsel and upon receipt of certificates of insurance, bonds and evidence that the contractor possesses a valid State Contractors License. Allow funds for contingencies in the amount of $159,991 (10%) to cover unexpected construction conditions and funds in the amount of $30,000 to cover construction engineering.

27.         Set a Public Hearing for July 21, 2015 at 9:30 a.m. in the Board Chambers, Administration Building, 2800 West Burrel, Visalia, California, to place delinquent water service fees, delinquent penalties and Auditor’s collection charges for Seville Water Company on the 2015/2016 Tax Roll. Approve the Notice of Public Hearing. 

28.         Set a Public Hearing for July 21, 2015 at 9:30 a.m. in the Board Chambers, Administration Building, 2800 West Burrel, Visalia, California, to place delinquent water and sewer service fees, penalties and Auditor’s collection charges in County Service Areas No. 1 and No. 2 on the 2015/2016 Tax Roll.  Approve the Notice of Public Hearing.

29.         Set a Public Hearing for July 21, 2015 at 9:30 a.m. in the Board Chambers, Administration Building, 2800 West Burrel, Visalia, California, to place delinquent sewer service fees, delinquent penalties and Auditor’s collection charges for the Terra Bella Sewer Maintenance District, on the 2015/2016 Tax Roll.  Approve the Notice of Public Hearing.

30.         Approve an amendment to Agreement No. 26176 with the United States Army Corps of Engineers for law enforcement services at Lake Kaweah Recreation Area in the amount of $36,637, retroactive to April 23, 2015 through April 21, 2016.  Find that the Board had authority to enter into the proposed amendment to the Agreement as of April 23, 2015, and that it was in the County’s best interest to enter into the agreement on that date.

31.         Approve an amendment to Agreement No. 25630 with the United States Army Corps of Engineers for law enforcement services at Success Lake Recreation Area in the amount of $47,311, retroactive to May 1, 2015 through April 30, 2016.  Find that the Board had authority to enter into the proposed amendment to the agreement as of May 1, 2015, and that it was in the County’s best interest to enter into the amendment to the agreement on that date.

32.         Approve an amendment to Agreement No. 26189 with Microcorre Diagnostic Laboratory to extend the term of the agreement to June 30, 2017 in the amount of $264,000 for providing pathology services.

33.         Approve an agreement with Canteen of Fresno, Inc. to provide commissary services for the period of July 1, 2015 and extending through June 30, 2017.
