5.                  PUBLIC HEARING:  Request from the Resource Management Agency to hold a Public Hearing to review and receive public comment on the Annual Grantee Performance Report for Grant No. 12-CDBG-8419.  Approve the Annual Grantee Performance Report.

6.                  PUBLIC HEARING:  Request from the Resource Management Agency to hold a Public Hearing regarding consideration of Mitigated Negative Declaration and Tentative Tract Map No. TM 812 with Exceptions, submitted by R-7 Enterprises/Dave Roberts (Agent: Donna Shamley/Roberts Engineering), for the division of 225 acres into 162 single-family residential lots, ranging in size from 22,500 sq. ft. to 98,650 sq. ft., in the PD-F-M (Planned Development-Foothill Combining-Special Mobilehome) Zone, located on the west side of Millwood Road, one mile northwest of Woodlake, generally described as a portion of Section 17, Township 21 South, Range 29 East, MDB&M.  Uphold the recommendation of the Planning Commission to deny the Mitigated Negative Declaration and Mitigation and Monitoring Plan based on the findings set forth in Planning Commission Resolution No. 8948.  Uphold the recommendation of the Planning Commission to deny the proposed tentative tract map (TM 812) based on the findings set forth in Planning Commission Resolution No. 8948.

Agricultural Commissioner/Sealer

7.                  Approve an agreement with the California Department of Food and Agriculture in the amount of $1,160,000 for an area wide Glassy-winged Sharpshooter multiyear management program, retroactive to July 1, 2014 through June 30, 2016.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014, and that it was in the County's best interest to enter into the agreement on that date.

8.                  Approve an agreement with the California Department of Food and Agriculture in the amount of $554,506 for the multiyear detection and suppression of Glassy-winged Sharpshooter (GWSS) program, retroactive to July 1, 2014 through June 30, 2016.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014, and that it was in the County's best interest to enter into the Agreement on that date.

Auditor-Controller/Registrar of Voters/Treasurer-Tax Collector

9.                  Approve Alpine Village-Sequoia Crest Community Services District's request to replace their annual audit with a biennial audit.

Board of Supervisors

10.             Appoint Dr. Roger A. Johnson, Historical Society Member to the Tulare County Museum Board for a term ending February 13, 2016.

11.             Reappoint Rebecca Donabed, Seat 3 - Consumer to the In Home Support Services Advisory Committee for a term ending June 30, 2017.

Capital Projects

12.             Approve the bid documents for the Bartlett Park Electrical Remodel Project. Authorize the advertisement of bids for the subject project and set the bid opening for the project at 2:00 p.m. on Thursday, September 11, 2014.

County Administrative Office

13.             Approve an amendment to Agreement No. 25787 with Hellmuth, Obata & Kassabaum, Inc. for additional architectural, design, and engineering services to increase the amount of the agreement by $168,000 to a total of $5,523,000 retroactive to May 12, 2014.  Find that the Board had the authority to enter into the proposed agreement as of May 12, 2014, and that it was in the County's best interest to enter into the agreement on that date.  Approved as to form by County Counsel.

14.             Approve a license agreement for office space with the Administrative Office of the Courts in the amount of $20,220 per year, and increasing 3% per year, for the use of office space in the Porterville Courthouse for the period commencing August 1, 2014, and continuing until terminated.

Health & Human Services Agency

15.             Reaffirm Resolution No. 2014-0090 declaring a Local Emergency due to the Drought in Tulare County adopted February 4, 2014.

16.             Approve an agreement with Turning Point of Central California, Inc. for the North County Mobile Unit, in an amount not to exceed $850,000, to provide mental health services retroactive to July 1, 2014 through June 30, 2015.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014, and that it was in the County's best interest to enter into the agreement on that date.

17.             Approve an agreement with ExMed, Inc. in an amount not to exceed $300,000 for adult, adolescent, and child psychiatric services in specific Tulare County Mental Health Clinics retroactive to July 1, 2014 through June 30, 2015.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014 and that it was in the County's best interest to enter into the agreement on that date.

18.             Approve an agreement with Victor Treatment Centers, Inc., in an amount not to exceed $110,000, for the provision of Intensive Day Treatment services (full day) to Tulare County minors retroactive to July 1, 2014 through June 30, 2015.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014, and that it was in the County's best interest to enter into the agreement on that date.

19.             Approve an agreement with Tulare Youth Service Bureau, Inc. in an amount not to exceed $177,012, to provide for the Maternal Mental Health Program retroactive to July 1, 2014 through June 30, 2015.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014, and that it was in the County's best interest to enter into the agreement on that date.

20.             Approve an agreement with Helios Healthcare, LLC, in an amount not to exceed $70,000 for the provision of acute psychiatric inpatient services for Medi-Cal beneficiaries retroactive to July 1, 2014 through June 30, 2015.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014, and that it was in the County's best interest to enter into the agreement on that date.

Human Resources & Development

21.             Approve a Memorandum of Understanding between the County of Tulare and the Service Employees International Union, Local 521, Bargaining Units 1, 3, 4, 6, & 7 retroactive to July 1, 2014 and continuing through June 30, 2015.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014, and that it was in the County's best interest to enter into the agreement on that date.

Purchasing

22.             Approve an agreement with the Tulare Local Healthcare District - Mineral King Toxicology Laboratory, for two (2) years from date of approval by Board with three (3) one (1) year extensions, to perform drug screenings, analysis and testing for County departments.

Resource Management Agency

23.             Review and accept the Commercial River Rafting Annual Report.  Authorize five rafting companies that were issued Commercial River Licenses in Calendar Year 2014 to maintain their status and be offered the opportunity to apply for one of the eight permitted spots in the 2015 Calendar Year for Commercial River Licenses.

24.             Approve an operation and maintenance agreement with the City of Tulare for a portion of Akers Street (Road 100 and Diagonal 103) north of Cartmill Avenue (Avenue 248) to a point 2,640 feet north of Cartmill Avenue, subject to review and approval as to form by County Counsel.

25.             Approve the designation of a One-Way STOP on Road 40 at Avenue 368 as a controlled intersection.  Authorize the installation of a STOP sign on Road 40 at Avenue 368 in accordance with the provisions of Section 2B.06 of the 2012 California Manual on Uniform Traffic Control Devices and Section 22450(c) of the California Vehicle Code.

26.             Approve the designation of a One-Way STOP on Canal Drive at Avenue 368 as a controlled intersection.  Authorize the installation of a STOP sign on Canal Drive at Avenue 368 in accordance with the provisions of Section 2B.06 of the 2012 California Manual on Uniform Traffic Control Devices and Section 22450(c) of the California Vehicle Code.

27.             Approve the designation of a One-Way STOP on Jacobs Drive at Canal Drive as a controlled intersection.  Authorize the installation of a STOP sign on Jacobs Drive at Canal Drive in accordance with the provisions of Section 2B.06 of the 2012 California Manual on Uniform Traffic Control Devices and Section 22450(c) of the California Vehicle Code.

Solid Waste

29.             Approve an agreement with Peña's Disposal Service to provide construction and demolition waste recycling services at the Tulare County Landfills until July 31, 2016, subject to approval as to form by County Counsel.

30.             Approve an amendment to Agreement No. 25073 with Shamrock Recycling of California, Inc., to extend the term of the agreement to June 30, 2015, retroactive to July 1, 2014 through June 30, 2015, in order to continue collection of waste tires and maintain the level of customer service to residents.  Find that the Board had authority to enter into the proposed agreement as of July 1, 2014, and that it was in the County's best interest to enter into the agreement on that date.

31.             Approve payment for Fiscal Year 2013/2014 purchase orders for capital assets approved in Fiscal Year 2013/2014 which will not be received until Fiscal Year 2014/2015.
